About The Position

Every day, we have the privilege of supporting individuals and families facing serious illness while helping our communities better understand caregiving, dementia, hospice, and palliative care. We are seeking a Community Partnership Liaison who is passionate about building relationships, educating others, and strengthening partnerships throughout the communities we serve. In this role, you will serve as a visible ambassador for Mountain Valley while developing relationships with community organizations, churches, senior centers, assisted living communities, skilled nursing facilities, healthcare professionals, and other referral partners. You will help increase awareness of our services through education, outreach, and meaningful community partnerships.

Requirements

  • Bachelor's degree in Communications, Marketing, Public Health, Healthcare Administration, Social Work, Education, Business, or a related field preferred.
  • Equivalent experience in healthcare outreach, community relations, nonprofit leadership, sales, marketing, ministry, or relationship management may be considered.
  • Strong communication, presentation, and interpersonal skills.
  • Demonstrated ability to build and maintain professional relationships.
  • Self-motivated with strong organizational skills and the ability to work independently.
  • Valid driver's license and ability to travel throughout Mountain Valley's service area.

Nice To Haves

  • Experience in healthcare, aging services, community outreach, or business development preferred.

Responsibilities

  • Build and maintain relationships with community organizations, healthcare providers, and referral partners.
  • Serve as Mountain Valley's representative at community events, educational programs, and networking activities.
  • Conduct outreach to assisted living communities, skilled nursing facilities, physician offices, and other community partners.
  • Present educational programs related to caregiving, dementia, advance care planning, hospice, and serious illness care.
  • Connect individuals, families, and community partners with available resources and support services.
  • Identify opportunities to develop new partnerships that support Mountain Valley's mission and strategic goals.
  • Collaborate with clinical, volunteer, patient advocacy, and growth teams to strengthen community connections and organizational outreach efforts.
