Medavie Health Services West (MHSW) is seeking a Community Paramedic (CP) to join their team in Moose Jaw. The CP plays a key role in promoting health and preventing illness, supplementing existing Primary Health Care services. This role is responsible for identifying health needs and risks, developing and implementing health programs, providing direct patient care, and collaborating with other healthcare professionals to improve access to care and health outcomes for youth in the community. As part of the Mobile Integrated Health Team, the Community Paramedic works closely with YMCA Homebase staff and supports a diverse patient population, including youth requiring mental health and addiction services, physical health supports, and coordination of care and community-based services. The CP works collaboratively within an interdisciplinary framework alongside service providers such as Community Mental Health and Addictions, Primary Health Care, Psychiatry, Paramedic Services, justice and victims’ services, child and family services, police services, and other community partners.
