About The Position

Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. We support health plans, health systems, and value-based organizations by reaching members where they are — in their homes and communities — to improve access, close care gaps, and reduce avoidable utilization. As a tech-enabled, field-based care delivery organization, our teams provide a range of services including preventive care, chronic condition support, transition-of-care visits, member engagement, and navigation. We operate with a focus on high-quality member experience, operational excellence, and coordinated care across clinical, administrative, and remote teams.

Requirements

  • Current Ohio Paramedic certification in good standing
  • Minimum 2–3 years of paramedic field experience
  • Valid driver’s license with acceptable driving record
  • Strong clinical assessment and patient education skills
  • Experience using electronic documentation systems

Responsibilities

  • Conduct scheduled patient visits within community-based care programs.
  • Perform comprehensive patient assessments including vital signs, symptom evaluation, medication reconciliation, and environmental safety review.
  • Deliver care within established clinical protocols and standing orders.
  • Provide treatments such as wound care, medication administration, and point-of-care testing when applicable.
  • Identify early signs of clinical deterioration and escalate concerns through clinical or operational pathways.
  • Support continuity of care following hospital discharge or acute medical events.
  • Assist patients in understanding care plans and follow-up appointments.
  • Identify barriers to recovery including environmental or social determinants of health.
  • Collaborate with care coordination teams to support patient stabilization and long-term health management.
  • Complete accurate and timely documentation within the electronic patient care record system.
  • Ensure documentation supports clinical accuracy, medical necessity, and payer requirements.
  • Maintain communication with operational teams regarding visit updates and patient needs.
  • Participate in documentation reviews and quality improvement initiatives.
  • Perform other related duties as assigned.
