About The Position

We are seeking a compassionate and skilled Community Paramedic to support a grant-funded initiative focused on improving patient outcomes, reducing hospital readmissions, and enhancing access to preventative and community-based healthcare services. This role provides non-emergency, patient-centered care in home and community settings, working closely with healthcare providers, social services, and community partners to address gaps in care and improve overall population health.

Requirements

  • Current State of Nebraska Paramedic License
  • Current State of Nebraska Community Paramedic License
  • Current Valid Driver’s License with an acceptable motor vehicle record
  • Strong assessment, clinical, and critical thinking skills
  • Ability to work independently in community-based settings
  • Excellent communication and interpersonal skills
  • Current National Registry of Emergency Medical Technicians Paramedic Certificate
  • International Board of Specialty Certification Community Paramedic Certification (CP-C)
  • American Heart Association BLS, ACLS, PALS provider required
  • NIMS IS100, IS200 required
  • 1 year experience at the paramedic level with an Advanced Life Support Service with a total call volume of 1500 or more calls per year. Or 2 years or more experience at the paramedic level with an Advanced Life Support Service.
  • Strong attention to detail and accuracy
  • Ability to manage multiple tasks and interruptions in a fast-paced environment
  • Basic computer proficiency and ability to learn electronic health record systems
  • Ability to maintain confidentiality and handle sensitive information
  • Commitment to quality, safety, and patient-centered care

  • NRP provider preferred
  • NIMS IS 700 and 800 preferred

Responsibilities

  • Conduct thorough in-home assessments, checking vital signs, reviewing medications, and addressing immediate concerns.
  • Provide treatments or reinforce care plans established after a recent hospital discharge.
  • Collaborate with physicians, nurses, case managers, and social workers to ensure coordinated and responsive patient care plans.
  • Identify social determinants of health (e.g., transportation, food insecurity, housing) and connect patients with appropriate community resources.
  • Educate patients on chronic disease management, medication adherence, and preventative care strategies.
  • Monitor patient progress and document each interaction in detail for continuity of care, clinical standards, and grant reporting.
  • Reduce unnecessary emergency room visits and hospital readmissions through proactive approaches and timely interventions.
  • Collect and report data to measure outcomes and demonstrate the effectiveness of the community paramedicine model.
  • Ensure all care provided aligns with clinical protocols, safety standards, and grant compliance.
