The Community Paramedic / Prehospital Registered Nurse (PHRN) provides advanced out-of-hospital patient care, health assessments, care coordination, disease-management education, wellness interventions, and EMS response services. The role connects emergency medical services with primary care providers, hospitals, public health agencies, and community resources to improve outcomes, reduce avoidable emergency department use, and expand access to healthcare. The CP/PHRN works independently within established protocols, physician orders, state regulations, and organizational policies, providing patient-centered care in homes, community settings, long-term care facilities, and during emergency responses. The position conducts comprehensive in-home assessments, medication reconciliation, patient education, chronic-condition monitoring, hospital discharge follow-up, fall-risk assessments, home safety evaluations, preventive health education, and wellness screenings. It identifies social determinants of health and connects patients with appropriate community resources. The role collaborates with emergency department staff, primary care providers, specialists, social workers, case managers, behavioral health professionals, public health agencies, home health providers, and other healthcare team members. It participates in multidisciplinary care planning, develops individualized plans for high-risk patients, and supports population health and quality improvement initiatives. The work aims to reduce preventable readmissions and emergency department utilization while improving continuity of care and access to community-based services. The CP/PHRN maintains compliance with applicable federal, state, and local regulations and protects patient confidentiality in accordance with HIPAA. The position participates in performance improvement, maintains required certifications and continuing education, and contributes to the development and enhancement of EMS and Community Paramedicine programs.
