About The Position

Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. The Community Paramedic provides in-home clinical care as part of TCC’s Mobile Integrated Healthcare programs, focusing on proactive care for high-risk populations, reducing hospital readmissions and emergency department use, and improving chronic disease management. The role involves scheduled patient visits, clinical assessments, care delivered under established protocols, and collaboration with virtual providers and interdisciplinary teams to provide coordinated, patient-centered care. The position is field-based, with care provided in patient homes, partner offices, or community clinical settings.
