About The Position

Under the direction of the Director of Clinical Services, the Community Outreach Worker (COW) is responsible for assisting Women’s Health Services staff in providing patient support for those in need of or qualified for women’s health related services. The COW will participate in activities that include but are not limited to, educating the community about women’s health services, family planning, cancer prevention, and other related needs in clinical and community settings, completing health related screenings, and assisting with linkage to care and services for people with identified medical and socio-economic needs. The Community Outreach Worker represents and attends local health fairs and community events. The Community Outreach Worker must be able to work in conjunction with the Women’s Health RN Case Manager, clinical staff, organizations, and agencies to identify and address needs. The demonstration of knowledge and skills to appropriately communicate and interact with staff, patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs must be exhibited by the Community Outreach Worker. This is a grant-funded position and continued employment is dependent on the availability of grant funds.

Requirements

  • High School diploma, Associate Degree, or experience in a related field preferred
  • Experience in health education/promotion
  • Valid Mississippi Driver’s license
  • Must be willing to travel locally, state-wide, and nationally
  • MS Office and typing skills required
  • Excellent oral and written communication skills including preparing reports, presentations, correspondence, and other documentation
  • Ability to handle on-demand requests and deadline pressure
  • Ability to work without supervision while handling workload
  • Must be able to pass a criminal background check, drug screen and TB skin test.

  • Fluent in English and Spanish preferred

Responsibilities

  • Identifies and addresses social determinants of health and other psychosocial barriers that impede people in need of women’s health and pregnancy related services
  • Participates in meetings of community groups, local organizations, professional groups, and regulatory agencies to promote SEMRHI and its services including but not limited to the Breast and Cervical Cancer Program (BCCP), prenatal care and related services, and family planning.
  • Actively participates in meetings with the women’s health staff to ensure coordination of services.
  • Effectively organizes and/or attends outreaches in varies community settings, such as schools, universities, community-based organizations (CBOs), and clinical settings, etc., to provide education to the targeted populations.
  • Completes home visits as needed to complete health related screenings, safety and needs surveys, and education regarding related services and resources.
  • Educates the community about family planning, breast and cervical cancer, and other women’s health related services.
  • Completes accurate data entry for all program related services and reports data to the Women’s Health RN Case Manager.
  • Documents face-to-face, telephone, screening, and other patient-related encounters in within 72 hours of the date of service.
  • Prepares and distributes flyers, brochures, and program education and prevention documents to educate the targeted population.
  • Communicates effectively and timely with the Director of Clinical Services and the Women’s Health RN Case Manager in the scheduling of speaking engagements, community presentations and other community outreach activities.
  • Represent SEMRHI on local and/or state level committees as requested by Director of Clinical Services.
  • Prepares accurate monthly reports by the 10th of each month when requested by the Director of Social Services or the Women’s Health RN Case Manager
  • Demonstrates positive personal attributes such as honesty, integrity, and professionalism in all aspects of the job.
  • Maintains prompt and regular attendance.
  • Complete all required training prior to deadlines
  • Performs related work, as assigned.
