About The Position

Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within an existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals. This Program works within an Assertive Community Treatment Model and utilizes the Housing First Principles from a social justice orientation which includes harm reduction, pro-choice, social determinants of health, human rights, social development, population health, health equality and trauma informed, client centered practice.

Requirements

  • Must have a Valid Driver's License and access to a reliable vehicle.
  • Minimum of 2 years’ experience working in community, preferably with program target population
  • Experience providing outreach with hard to reach populations
  • Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management
  • Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
  • Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values
  • Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families
  • Proven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity and cultural awareness
  • Strong knowledge, commitment and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed and community development principles in all aspects of the work
  • Excellent verbal and written communication skills and proficiency in the use of computer systems

  • Post-Secondary Education in a Social Services field
