About The Position

The Community Outreach Specialist – Private Pay will be responsible for expanding Always Compassionate Health's private pay services throughout Long Island by developing relationships with prospective clients, families, healthcare professionals, senior communities, professional organizations, and other community partners. Reporting directly to the Director of Private Pay, this position will focus on generating private pay leads and referrals, developing and maintaining referral relationships, increasing awareness of Always Compassionate Health's private pay services, and supporting the conversion of qualified prospects into active clients. The position is based in Melville, NY and requires approximately 75% local travel throughout Long Island.

Requirements

Experience in community outreach or a related field.
Strong relationship-building and communication skills.
Ability to travel approximately 75% throughout Long Island.
Proficiency in maintaining accurate records.

Responsibilities

Develop and execute community outreach strategies to increase private pay referrals, leads, and new client opportunities.
Build and maintain strong relationships with senior living communities, assisted living facilities, hospitals, rehabilitation centers, physicians, social workers, care managers, elder law professionals, financial advisors, community organizations, and other referral sources.
Identify and develop new referral sources and community partnerships that support private pay growth.
Conduct regular in-person visits with current and prospective referral partners throughout Long Island.
Meet with prospective private pay clients and their families to understand their needs and educate them about Always Compassionate Health's services.
Clearly communicate the benefits and value of private pay home care services.
Organize and participate in community events, senior expos, health fairs, educational seminars, networking events, and other outreach opportunities.
Research local market trends, demographics, competitors, and community resources to identify opportunities for private pay growth.
Collaborate with the Director of Private Pay, Intake, Operations, and other internal departments to facilitate a smooth transition from initial inquiry through client enrollment.
