About The Position

Responsible for assisting eligible low-income families for the Healthy Kids Grant Project. For families already enrolled in Kancare, responsible to assist families in maintaining coverage. Responsible for traveling to outreach locations in order to meet with families in-person on a routine basis.

Requirements

  • High School Diploma or GED required.
  • Must have valid driver’s license and reliable transportation for commuting to outreach events
  • Minimum of at least 2 years in social work or community outreach.
  • Bilingual (Spanish) required.
  • Must possess a thorough knowledge of modern office practices, procedures, and equipment, including computers, copiers, and other standard office equipment.
  • Ability to work independently, organize, monitor, and adjust work as necessary to ensure accuracy and timeliness.
  • Ability to strongly embrace and personify the mission and values of GraceMed with socio-economic and cultural sensitivity in mind.
  • Ability to meet people with ease; ability to write and edit effectively; ability to communicate the written and spoken word with tact, diplomacy, and/or authority when necessary.
  • Possess effective organizational skills; well-disciplined, able to meet deadlines, self-starter, able to work under pressure of many priorities and deadlines.
  • Must possess knowledge of Kancare program.
  • Must have highly developed interpersonal skills.
  • Adheres to Agency Policy and Procedures.
  • Acts as a role model within and outside the Agency.
  • Performs duties as required.
  • Maintains a positive and respectful attitude.
  • Communicates regularly with the Outreach Project Coordinator and Director of Outreach and Eligibility.
  • Demonstrates flexible and efficient time management and ability to prioritize workload.
  • Consistently reports to work on time; prepared to perform duties of the position.
  • Meets defined goals and objectives.
  • Must interact with professionals, clients, patients, and other office personnel.
  • Must possess socio-economic and cultural sensitivity.
  • May involve contact with angry, upset or ill persons.
  • Must have visual acuity and manual dexterity to interface with a computer.
  • Must have auditory acuity to handle phone calls and extensive interaction with the public.

Responsibilities

  • Engage in strong outreach activities reaching out into the 29 county service area.
  • Educate families about the availability of free or low-cost health coverage under Medicaid and CHIP.
  • Identify children and families likely to be eligible for these programs and assisting families, including parents to apply for coverage.
  • Provide support for renewal/retention of health coverage benefits.
  • Facilitate connections with appropriate local resources in order to assist the children/families with enrollment in appropriate health insurance option or provide assistance to families with online self-enrollment options as needed.
  • Accomplishes additional tasks as appropriate.
