About The Position

Southwest Behavioral & Health Services is seeking a Community Outreach Navigator to join their team at the Avondale Resource Center. This role involves working in the office Monday-Friday during standard business hours. The Community Outreach Navigator serves as a central support for individuals of a wide range of ages, providing crisis intervention, one-on-one support, psycho-social prevention education, and trauma-informed training to staff. The staff member is responsible for assisting individuals in developing and implementing their personal wellness program through support, education, and motivation.

Requirements

  • Minimum of 21-year of age
  • Bachelor’s degree in related field plus one year of relevant experience
  • Valid AZ driver’s license, and Motor Vehicle clearance
  • Ability to obtain fingerprint clearance card

Responsibilities

  • Provides one-on-one community-based support to a caseload of members
  • Leads psycho-social prevention & wellness groups
  • Assists with crisis intervention with walk-in visitors
  • Administers trauma-informed training to team at Resource Center
  • Works collaboratively with team to support individual service plans
  • Provides accurate documentation of contact with members
