About The Position

Under the supervision of the Director of Development and Community Engagement, the Community Outreach Manager provides leadership and direct execution of Kinston Community Health Center’s outreach, patient navigation, and mobile health services. This role is responsible for coordinating non-clinical mobile unit operations, supervising outreach staff, and ensuring patients are effectively connected to medical, behavioral health, and social support services. This is a hands-on leadership role that requires active participation in outreach and Community Health Worker (CHW) functions. The Community Outreach Manager maintains a caseload, engages directly with patients and community partners, and supports mobile unit activities in the field—while also overseeing team performance and program operations. The position plays a critical role in improving access to care, addressing social determinants of health, and advancing organizational goals through both strategic leadership and direct service delivery.

Requirements

  • Bachelor’s degree preferred in: Social Work, Public Health, Human Services, Health Administration or related field
  • Equivalent experience may be considered
  • Must obtain Community Health Worker (CHW) certification within 6 months of hire
  • Valid North Carolina driver’s license with acceptable driving record
  • Must be insurable under the organization’s vehicle insurance policy
  • Ability to safely operate a range of organizational vehicles, including: Standard passenger vehicles, Passenger vans, Large mobile medical units (RV-style)
  • 3–5+ years in: Community outreach, Patient navigation or case management, Public health or healthcare support services
  • Experience in healthcare, nonprofit, or community-based organizations.
  • Strong understanding of social determinants of health
  • Ability to manage programs, staff, and logistics simultaneously
  • Experience with EHR systems (NextGen preferred)
  • Ability to build and maintain community partnerships
  • Ability to operate mobile unit systems from an operational (non-clinical) standpoint
  • Strong written and verbal communication skills.
  • Ability to manage multiple projects and deadlines simultaneously.
  • Strong organizational and project coordination skills.
  • Ability to analyze data and identify opportunities for improvement.
  • Ability to work collaboratively with internal departments and community partners.
  • Proficient in Microsoft Office Suite

Responsibilities

  • Oversee day-to-day operations and deployment of the mobile medical unit, ensuring readiness for all scheduled events
  • Coordinate site logistics including scheduling, location setup, equipment readiness, and supply management
  • Serve as primary liaison with community partners and host sites for mobile unit activities
  • Operate and/or oversee operation of mobile units, vans, and other organizational vehicles
  • Ensure completion of vehicle inspections, maintenance logs, and safety documentation
  • Monitor and ensure compliance with all transportation, safety, and organizational policies
  • Collaborate with clinical staff to ensure efficient, patient-centered workflows during mobile deployments
  • Assumes responsibility for safe operation and transport of the mobile medical unit as needed to support program delivery
  • Oversee and support all patient navigation activities to ensure timely access to medical, behavioral health, and social services
  • Maintain an active caseload and function as a Community Health Worker (CHW)
  • Conduct outreach activities including community events, home visits, and health education initiatives
  • Assess and address social determinants of health, including food insecurity, housing, transportation, and insurance coverage
  • Connect patients to internal services and external community resources, ensuring appropriate follow-up
  • Support patient registration, scheduling, and coordination of care across departments
  • Plan, implement, and evaluate outreach initiatives aligned with organizational goals
  • Monitor key performance indicators (KPIs) related to outreach, navigation, and mobile unit services
  • Ensure accurate, timely, and compliant documentation in the electronic health record (EHR) system
  • Prepare and submit required reports for leadership, grants, and regulatory agencies
  • Identify gaps in service delivery and implement process improvements to enhance efficiency and outcomes
  • Support strategic growth of outreach programs and mobile health services
  • Monitor and manage outreach-related supplies, resources, and operational needs
  • Provide input into budget planning and resource allocation for outreach and mobile services
  • Represent the organization at community events, health fairs, and partner meetings
  • Develop and maintain relationships with community organizations, agencies, and stakeholders
  • Identify opportunities to expand outreach efforts and increase access to care
  • Collaborate with internal departments to align outreach initiatives with organizational priorities
  • Promote awareness of services, programs, and resources available through the organization
  • Exercises independent judgment in coordinating outreach operations, prioritizing resources, and resolving operational challenges
  • Ensure all outreach, navigation, and mobile unit activities comply with HIPAA, HRSA, and organizational policies
  • Maintain confidentiality of patient and organizational information
  • Ensure staff adherence to documentation, safety, and compliance standards
  • Participate in audits, quality improvement initiatives, and training as required
  • Report and document incidents, safety concerns, or operational issues in accordance with organizational policy
  • Perform all other duties as assigned
  • Supervise Outreach Team staff, including Community Health Workers and Patient Navigators
  • Develop and manage staff schedules, assignments, and coverage for outreach and mobile unit operations
  • Monitor staff productivity, caseloads, and performance outcomes
  • Provide coaching, training, and ongoing professional development to team members
  • Conduct performance evaluations and address performance concerns as needed
  • Foster a collaborative, accountable, and mission-driven team environment

  • Promptly report any known or suspected violations of compliance/safety standards.
  • Must successfully pass required background checks in accordance with organizational policy.
