About The Position

This is a full-time (35 hr/wk) role based out of our Greenfield office, serving the Franklin County, and North Quabbin area. We are dedicated to the well-being of older adults, individuals with disabilities, and caregivers. The pillars of our work include inclusion, well-being, and strengthening our communities by supporting all individuals. The Community Options Resource Specialist is responsible for developing supportive relationships with older adults and people with a disability, fostering independence, and serving as a central source of support and resources. Responsibilities include dissemination of information and making appropriate referrals, creating a service plan for each consumer, and serving as an advocate.

Requirements

2-3 years of previous experience in health or human services required.
Willingness and sensitivity for working with older adults.
Strong documentation skills and familiarity with electronic health records (EHR) or care management platforms and Microsoft programs.
Ability to work independently and as part of an interdisciplinary team.
Exceptional interpersonal and communication skills
Capacity to complete comprehensive needs assessments using observation, listening, and interviewing skills.
Ability to solve problems and find solutions.
Flexible and able to meet multiple, and changing, program demands.
Familiarity with or willingness to learn about health and human services programs.
Demonstrates cultural humility and cultural responsiveness.
Visual, speaking, auditory, and mobile capacity necessary.
Capacity to see computer screen, read written material, and drive a car.
Capacity to hear and speak on the telephone.
Capacity to communicate verbally with consumers, caregivers, supervisors, and managers.
Capacity for fine manipulation in the frequent use of office equipment such as computers, copy machines, fax machines, telephones, calculators, etc.
Capacity to drive and navigate uneven terrain by foot when necessary in all weather conditions to visit consumers in their homes.
Capacity to climb stairs.
Ability to sit or stand for extended periods of time.
Occasional reaching and grabbing objects with both hands, twisting of hand and wrist, and pushing and pulling of objects.
Occasional bending, squatting, and twisting to perform work functions.
Occasional capacity to lift up to 25 pounds.
Ability to understand and/or interpret complicated program instructions, regulations, and laws.
Capacity to learn complicated computer software.
Capacity to deal rationally and calmly with varying personalities and situations.
Capacity to work well in a fast-paced, rapidly changing environment.
Must be able to tolerate odors and pollutants including but not limited to: smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.
Must be able to tolerate by-products of office machine operation.
Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.

Nice To Haves

Bachelor’s Degree preferred
License in Social Work is a plus.
6% pay differential for applicable bilingual skills upon satisfactory completion of fluency assessment.

Responsibilities

Conduct virtual and in-person comprehensive, person-centered assessments with consumers, care partners, and health-care professionals.
Develop, implement, and update a service plan with the consumer in collaboration with their formal and informal support systems.
Make appropriate referrals to other community-based agencies and governmental organizations.
Collaborate with internal and external providers to ensure adequate coordination of service plans (to include meetings and telephone contacts).
Adhere to documentation standards and maintenance of consumer records according to regulations.
Assist consumers, families, caregivers, and informal supports in problem-solving. Facilitate team meetings and case/family conferences as needed.
Maintain up-to-date knowledge of information about services available to older adults and people with disabilities in the service area.
Serve as an advocate for consumers to obtain other needed services.
Participate in interdisciplinary team meetings as needed.
Act as a representative of LifePath that times when directed to do so.
Provide coverage for the department as assigned.
Participate in staff meetings and trainings as required.
Assist in training new staff by allowing them to shadow visits and by sharing information and tips.
Other duties as assigned by immediate supervisor, other designated supervisory staff, or designated member of the Management Team.
