Requirements

  • Masters Degree or recognized equivalent in Occupational Therapy.
  • Registered and in good standing with provincial College of Occupational Therapists.
  • One year providing Occupational Therapy services and experience with in-home assessments and treatments is an asset.
  • Driver's license and reliable vehicle.
  • Maintain active standing in College of Occupational Therapists of Ontario.

Responsibilities

  • Assessment of clients, establishment and ongoing evaluation and revision of treatment programs, including goals and discharge plans.
  • Maintenance of current accurate clinical records according to standards set by the College of Occupational Therapists of Ontario, documenting assessment results, goals, treatment, client progress, and discharge plans.
  • Collaboration with other members of the health team as needed.
  • Communication of pertinent information and recommendations to clients/care providers and provision of education regarding treatment strategies as needed.
  • Observation of the performance of those assisting in the Occupational Therapy program and provision of guidance as necessary.
  • Completion of professional activities including assessment, treatment, education, referrals for service, and management of clients requiring social work intervention, in accordance with professional standards.
  • Participation in ongoing internal and/or external education activities.
  • Adherence to Bayshore Therapy & Rehab Policies and Procedures.
  • Participation in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participation in proactive Health & Safety activities while performing all duties and notification of any Health & Safety risks or concerns to the immediate Supervisor.
  • Maintenance of confidentiality of client and corporate information.
  • Completion of other tasks as requested.
