Community Nurse- Visiting Nurse
About The Position
Nurses working in the community experience flexible scheduling and work-life balance. They develop meaningful therapeutic relationships with patients while preserving patient dignity and privacy. Working within a supportive and collaborative environment focused on quality of patient care, community nurses can exercise their full scope of practice, pursue specialization, and champion innovative health care programs. This is a Temporary 6-month Full-Time position in the St. Thomas and surrounding areas. Hours of work are 8 am - 8 pm within a 2-week rotation inclusive of weekends, weekdays, and on-call work.
Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver’s license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- A current vulnerable sector check and a clear background check
Responsibilities
- Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
- Working with patients (both in-person and virtually) to provide exceptional nursing services.
- Providing services in a variety of settings which may include in-home, clinic, retirement home or school.
- Communicating and liaising with patients, family members and others within the interdisciplinary care team.
- Performing other clinical duties as required.
