Community Mental Health Rehabilitation Specialist
About The Position
Grace Community Care & Homes is developing its Mental Health department and seeks a Community Mental Health Rehabilitation Specialist. The role will contribute to person-centered services, reliable operations and accurate documentation. This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.
Requirements
- Education and mental-health rehabilitation experience appropriate to the final CSS staffing role; work under required program supervision.
- Applicants should provide relevant credentials, experience, availability and a resume.
- Final regulatory eligibility will be verified before appointment or service delivery.
Responsibilities
- Support individualized rehabilitation goals, daily living skills, community functioning and service connections; document activities under the approved CSS program.
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review and quality improvement.
