About The Position

A Community Living Support (CLS) professional provides one-on-one, in-home or community-based support to individuals with disabilities or special needs, aiming to increase their independence, safety, and community integration. Key duties include assisting with daily living tasks, such as cleaning, cooking, hygiene, and transportation, while following a personalized support plan. Age group 3 yrs to adult.

Requirements

  • High school diploma or GED is required.
  • Valid driver's license and a reliable vehicle with insurance, and a good driving record.
  • Must be able to obtain a valid chauffeur's license.
  • Obtain First Aid/CPR certification within 30 days of hire; training is provided in-house.
  • Have an acceptable driving record.
  • Have or be able to obtain a valid chauffeur's license.
  • Have a cell phone.

Nice To Haves

  • Experience working with individuals with disabilities is helpful.

Responsibilities

  • Develop Participant in, or assist with, activities of daily living (ADLs), including personal hygiene, cooking, laundry, and household maintenance.
  • Assist individuals in accessing the community, including transportation to social events, recreation, and medical appointments.
  • Maintain accurate, timely, and legible records, including daily session notes, progress reports, and behavior tracking data.
  • Implement goals outlined in the consumer's Person-Centered Plan (PCP) to promote independence.
  • Monitor health, remind or assist with medication, and ensure a safe, clean environment.
  • Offer encouragement and help develop social skills and relationships outside of staff.
