About The Position

The Community Liaison acts as a liaison between the Visiting Nurses Association (VNA) and the community regarding community and patient/client needs. This role involves the development and promotion of Home Health and Hospice programs and services, ensuring adherence to quality healthcare standards and optimal resource utilization. The liaison provides counsel on community needs, program development (including personnel and financial aspects), and collaborates on policy creation for new programs. A key aspect is maintaining compliance with all relevant laws concerning the marketing of Home Health and Hospice services. The position also involves assisting in program direction through collaboration and delegation, partnering with the Interdisciplinary Group/Team for safe patient/family care, and establishing positive relationships with referral sources, healthcare providers, potential patients, and community organizations. Representation of the VNA at community events is also a core responsibility.

Requirements

  • Graduation from high school or completion of GED required.
  • Bachelor's degree with minimum of one year experience in marketing OR minimum of three years' experience working in the areas of development and resource development, event management, marketing and public relations, preferably in health care or not for profit.
  • Basic level knowledge of EMR (electronic medical records), Microsoft operating system and Microsoft Word, Excel and Outlook.
  • Working knowledge of Microsoft Publisher and/or Adobe InDesign programs and Microsoft PowerPoint.
  • Experience with production and editing of media related material.
  • Criminal background check and motor vehicle (MVR) check required.

Responsibilities

  • Development and promotion of Home Health and Hospice programs and services.
  • Maintaining standards of practice consistent with quality health care and maximizing human, financial and equipment resources.
  • Providing counsel to the agency regarding the needs of the community; program development including personnel needs and financial needs.
  • Collaboratively developing and maintaining policies specific to new programs.
  • Ensuring compliance with all local, state and federal laws relating to the marketing of Home Health and Hospice.
  • Assisting in direction of programs through collaboration with, and delegation of responsibility to, administrative and supervisory personnel.
  • Partnering with the Interdisciplinary Group/Team to support safe and effective patient/family care.
  • Establishing and maintaining positive relationships and promoting VNA to referral sources, physician practices, hospitals, potential patients, civic groups, community businesses and service organizations.
  • Representing VNA at community activities and events such as; health and vendor fairs, open houses, ribbon cuttings, etc., to promote agency initiatives.
  • Carrying out other duties as assigned.
