The Community Liaison is responsible for responding to onsite inquiries regarding information about Catholic Hospice services and for coordinating the scheduling of referrals for an Admissions evaluation. This role ensures effective communication with Marketing, field Admissions, referral sources, and Interdisciplinary Teams. The liaison takes referrals from all sources, obtains written consent, verifies demographic information, and communicates essential details to the call center and Admissions Nurse. They also explain hospice services to patients/families at the bedside, provide accurate documentation, and coordinate with the call center to ensure timely admissions and explore reimbursement areas. This position participates in a seven-day-a-week coverage schedule, including evenings and holidays, and documents all relevant conversations. The Community Liaison supports Marketing Representatives, helps increase the referral base, and may serve as an interim department leader.
