As a Community Liaison, you will serve as the primary connection between our skilled nursing facility and the broader healthcare community. You’ll be the first point of contact for referral sources, families, and discharge planners—guiding tours, coordinating admissions, and building strong relationships that support census growth and continuity of care. You’ll also play a key role in following patients after discharge, ensuring smooth transitions and strengthening long-term partnerships with hospitals, physicians, and other healthcare providers. This is a highly visible, relationship-driven sales role that blends healthcare knowledge, communication, and community outreach. The Community Liaison supports skilled nursing census growth through proactive relationship management, referral development, and admissions coordination. This role works closely with facility leadership to position the community as a trusted post-acute care partner while upholding Americare’s mission, values, and Hometown Hospitality® standards. This role may also be known as: Community Liaison, Admissions Liaison, Marketing Liaison, Healthcare Sales Representative, or Referral Coordinator.
