About The Position

We are seeking an ambitious Community Liaison to dominate our Northern Colorado territory. Operating out of our central Windsor office, you will own your daily schedule, drive a company-provided vehicle, and build high-trust referral networks that directly impact the lives of seniors in our community. Your earning potential has no ceiling. On top of a competitive base salary, you will earn uncapped quarterly bonuses driven directly by your performance.

Requirements

Proven track record in Direct Sales; previous experience as a healthcare or home care Community Liaison is strongly preferred.
Bachelor’s degree in Business, Sales, Marketing, or a related field.
Strong interpersonal skills, strategic networking ability, assertiveness, and self-motivated schedule management.
Deep familiarity with or passion for serving the Northern Colorado healthcare landscape.

Responsibilities

Build Strategic Partnerships: Connect daily with Hospital Discharge Planners, Case Managers, Assisted Living Directors, Social Workers, and Primary Care Physicians.
Hit the Ground Running (First 90 Days): Focus heavily on cold outreach, networking, and rapport-building to lay the foundation for steady referral streams.
Drive Regional Sales: Manage local sales operations and execute creative outreach initiatives to capture market share across Fort Collins, Loveland, Greeley, Windsor, and surrounding communities.
Collaborate for Quality: Work closely with our internal clinical team to ensure seamless transitions of care and consistent brand messaging.
