About The Position

The Community Liaison serves as a key ambassador and patient advocate for VNA Hospice NWI. This position is responsible for developing and maintaining professional relationships with referral sources, educating healthcare partners about hospice services, and supporting organizational growth through increased referrals and admissions. The Community Liaison acts as the primary connection between VNA Hospice NWI and healthcare providers, hospitals, extended care facilities, physician practices, and community organizations.

Requirements

  • Minimum of 2 years of sales experience, preferably in healthcare, hospice, home health, or a related field.
  • Excellent written, verbal, presentation, and relationship-building skills.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the assigned service area.
  • Strong interpersonal and networking abilities.
  • Ability to build trust and maintain long-term professional partnerships.
  • Proven ability to meet growth and referral goals.
  • Strong organizational and time-management skills.

Nice To Haves

  • Bachelor’s degree in business, Healthcare Administration, Marketing, Social Work, Nursing, or a related field.
  • Healthcare experience in nursing, social work, hospice, home health, or a related healthcare setting.
  • Experience working with physicians, hospitals, discharge planners, case managers, or long-term care facilities.
  • CRM and database management experience preferred.
  • Knowledge of hospice services and the healthcare continuum of care.

Responsibilities

  • Cultivate, develop, and strengthen relationships with hospitals, extended care facilities (ECFs), physician offices, medical practices, and community agencies.
  • Expand referrals, admissions, and lengths of stay through strategic relationship-building with physicians, office staff, social workers, discharge planners, case managers, healthcare facilities, and community organizations.
  • Manage assigned territories/accounts to achieve census and growth objectives.
  • Communicate program outcomes, service effectiveness, and organizational updates to referral partners.
  • Stay informed on industry trends, market developments, and regulatory changes affecting hospice care.
  • Serve as a knowledgeable resource regarding VNA Hospice NWI's mission, values, services, resources, and eligibility requirements.
  • Understand and communicate the organization’s strategic goals, marketing initiatives, and competitive advantages.
  • Collaborate with the Marketing Manager and communications team to coordinate and deliver educational presentations, in-services, community events, and promotional activities.
  • Submit monthly activity and performance reports to management.
  • Utilize Referral Connect CRM to maintain accurate records and timely reporting.
  • Ensure referral source databases and contact information remain current and accurate.
  • Support the agency’s commitment to preventing fraud and abuse.
  • Maintain knowledge of hospice philosophy, state regulations, and federal compliance requirements.
  • Protect the confidentiality of patients, families, and team members.
  • Participate in ongoing professional development and educational opportunities.
  • Demonstrate flexibility, adaptability, and a positive approach to organizational change.
  • Perform additional duties as assigned.
