As a Patient Care Navigator (PCN), sometimes called a Community Liaison, or Community Marketer, you'll have a unique opportunity to solve more patient and referral partner problems with our incredible continuum of healthcare services. In this role you will strategically grow our business within your assigned territory, enhance existing partnerships, maintain connections with patients and their families, and generate new opportunities for Atlas Medical, Aegis Home Health, and Aegis Hospice. You'll be instrumental in achieving monthly goals and will actively participate in community networking events to forge new business relationships and deliver compelling Home Health and Hospice presentations. Additionally, your adaptability will be key, as you'll handle various related duties to support our team's success. Requirements

  • 2+ years Hospice Referral Relationship building and/or Marketing experience
  • Knowledge of Local Senior Care Industry
  • Intuitive, solution oriented, and driven to achieve goals
  • Detail oriented, flexible, and self-motivated
  • Excellent oral and written communication skills
  • Valid Driver's License (clean record) & reliable transportation
  • Proof of current auto insurance & car registration
  • Valid AZ Fingerprint Clearance Card (we can help)
  • Current CPR Certification (we can help) & Negative TB Test (we can test)
  • All Aegis Healthcare employees must be able to pass a criminal background check.
