About The Position

Youth Homes is Contra Costa’s leading provider of trauma-informed programs for foster youth. We focus on empowering current and former foster youth, young adults with complex behavioral health challenges, and under-resourced children at risk of entering the foster care system. JOIN OUR TEAM, BE THE DIFFERENCE! Youth Homes is seeking a Community Liaison who will join our team of dedicated providers to serve foster and at-risk youth. In this role you will deliver Intensive Home-Based Services (IHBS) to children and youth with emotional or behavioral challenges. The Liaison works across Contra Costa County in homes and community settings, providing strength-based interventions, supporting caregivers, and coordinating services through the Child and Family Team (CFT) framework. This position is part of Youth Homes’ continuum of care and emphasizes trauma-informed, culturally responsive, and community-embedded practices. The starting pay for this full-time, non-exempt position is $23.42 - $24.59 per hour depending on experience

Requirements

  • Associate's degree in behavioral science.
  • Demonstrates cultural humility and a strong community-based practice orientation.
  • Build effective relationships with families and community partners.
  • Collaborates within interdisciplinary teams.
  • Responds skillfully to crises with stabilization and de-escalation techniques.
  • Maintains an organized, reliable, and flexible work style to meet the dynamic needs of youth and families.
  • At least 21 years of age
  • Must complete criminal background check, health screening and other state/federal licensing/certification requirements.
  • Must have own/reliable transportation, personal auto insurance and a valid drvier's license with no restrictions.

Nice To Haves

  • Bachelor's degree in behavioral science
  • Experience working in youth and adolescent programs, or in school-based treatment programs
  • Bilingual (English/Spanish) skills

Responsibilities

  • Provide individualized, strength-based interventions to improve youth functioning in home, school, and community settings.
  • Build youth skills in self-regulation, communication, social engagement, academic achievement, and independent living.
  • Deliver services in flexible settings—family homes, foster homes, schools, and other community locations, with evening and weekend availability.
  • Collaborate with ICC Coordinators, therapists, child welfare, probation, school districts, and community-based organizations to ensure coordinated, integrated care.
  • Participate in Child and Family Team (CFT) meetings and contribute to the development and ongoing monitoring of individualized treatment plans.
  • Connect youth and families with relevant community resources, including housing support, recreational programs, tutoring, mentoring, and family resource centers.
  • Represent Youth Homes at meetings, community events, and in cross-agency collaboration.
  • Serve as a consistent field-based point of contact, maintaining engagement with families.
  • Complete timely and accurate documentation in compliance with Medi-Cal Specialty Mental Health Services (SMHS) standards.
  • Maintain up-to-date treatment plans, progress notes, and required Medi-Cal authorization materials.
