About The Position

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country. The Marketing team of BJC Home Care Services works to promote our organization's services to and facilitate patient referrals from physicians, hospitals and other healthcare facilities in the geographic area in which we provide patient care and services.

Requirements

• Bachelor's Degree
• 2-5 years of experience
• Automobile Liability Insurance
• Valid Driver's License

Responsibilities

• Builds and maintains effective relationships with key referral sources.
• Develops, facilitates, and coordinates the referral process to promote a seamless, timely and comfortable transition experience for referring providers, patients, and their families.
• Provides presentations of hospice philosophy and program requirements to an array of healthcare and community audiences.
• Identify areas to increase referrals and develop and implement marketing plans.
• Serves as a liaison between BJC Home Care Services and clients, actively participating and engaging in community partnerships.
