About The Position

CHOR Youth & Family Services is seeking a Community Interventionist to provide quality community interventions for adolescents and their families in Lehigh and Northampton Counties. The primary responsibilities include acting as a liaison between The Children's Home of Reading and referral/other agencies, representing the agency at court hearings and interagency meetings, identifying and utilizing community services/supports, acting as an advocate and mentor, assisting with discharge planning, and transporting clients.

Requirements

  • Must be at least 21 years of age.
  • Valid PA driver’s license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage.
  • At least three (3) written references that demonstrate past educational and/or employment experience.
  • Successful completion of core development orientation program.
  • PA Act 33/34/73 clearances and proof of exclusions.
  • Must meet all requirements of Title 8, United States Code, Section 1324-A of the United States Department of Justice/Immigration and Naturalization Services Employment Eligibility Verification.
  • Pre-employment physical from a licensed physician (after conditional job offer) verifying person has no communicable diseases or physical conditions that might endanger the health and safety of children or prevent the person from performing essential job responsibilities.
  • All applicants must undergo pre-employment drug screening, which includes testing for marijuana, as well as background checks (including FBI, State and ChildLine clearances).

Nice To Haves

  • Bachelor’s degree in social work, psychology or other related field preferred.
  • One to three years experience preferred working in a human services field (CASSP system-related setting preferred).

Responsibilities

  • Act as a liaison and maintain communication between The Children's Home of Reading, referral/other agencies.
  • Represent the agency at all court hearings and interagency meetings.
  • Identify and utilize community services/supports as needed.
  • Act as an advocate.
  • Act as a mentor.
  • Assist with discharge planning.
  • Transport clients.
  • Facilitate/co-facilitate groups for adolescents.
  • Complete all required documentation including service plans/reviews, weekly summaries and discharge summaries.
  • Assist families with obtaining/maintaining basic entitlements and benefits as necessary.
