About The Position

The Arc Prince George’s County serves over 600 individuals with disabilities through various programs and services, focusing on community living, family support, employment services, and day programs. The Community Integration Specialist will provide essential support to help individuals achieve their desired lifestyles, increase independence, productivity, and social/physical inclusion. This role involves supporting the person-centered planning process, transporting individuals to activities, supervising daily program activities, and completing necessary documentation.

Requirements

  • High school diploma or equivalent.
  • 18 years of age.
  • Desire to work with adults who require a great deal of hands-on assistance, personal care and supervision.
  • Computer literate.
  • Excellent oral and written skills.
  • Strong communication skills.
  • Dependable and reliable.
  • Basic computer skills and ability to use all required software.
  • Ability to problem-solve and assist in decision-making, including during emergencies.
  • Maintain current status regarding assigned core competencies and other required certification acquisition within established schedules and timelines.
  • Meet the criteria to be an approved driver for The Arc of Prince George’s County.
  • Reliable transportation.
  • Ability to work productively with individuals and other team members to develop relationships.
  • Excellent interpersonal skills.
  • Good customer service skills.

Responsibilities

  • Develop and assist with the person-centered planning process.
  • Transport individuals supported to and from home and inclusive community activities.
  • Supervise and implement daily program activities and community experiences.
  • Complete necessary daily and follow-up documentation.
  • Plan for change, listen to individuals' interests, and teach essential community skills.
  • Create or review Person-Centered Plans (PCPs) to ensure alignment with preferences and goals.
  • Help individuals identify and plan community experiences based on their needs and interests.
  • Implement Person-Centered Plans for inclusive activities and skill development.
  • Provide support for community engagement and teach life skills.
  • Foster positive work habits.
  • Assist individuals with daily living activities and provide constant supervision.
  • Ensure personal and health care needs are met.
  • Maintain a safe environment.
  • Respond to medical emergencies and administer medication accurately.
  • Adhere to safety protocols and report incidents.
  • Complete accurate documentation, attend required meetings and trainings.
  • Support policies and the agency’s mission.
  • Promote teamwork and person-centered planning.
  • Actively participate in Person-Centered Plan development.
  • Manage challenging behaviors.
  • Maintain effective communication with team members.
  • Ensure compliance with regulations and agency standards.
