About The Position

Lakeview is seeking a Community Integration Specialist for its Millpond Location in Westfield, WI. This is a full-time, day shift position that offers a $5,000 sign-on bonus. The role involves providing assistance and treatment to program participants, ensuring their active participation in the rehabilitation program, and serving as a member of the interdisciplinary treatment team. Responsibilities include providing education, assistance, supervision, safety, and behavior management for program participants.

Requirements

  • CNA (Certified Nursing Assistant) Certification
  • RA (Resident Assistant) Certification
  • Knowledge of the sequela of brain injury and other neurological disorders.
  • Knowledge of physical, cognitive, psychological, emotional, and/or communication disabilities.
  • Knowledge and experience with basic care giving.

Nice To Haves

  • CBRF certifications preferred which include: Medication Administration, Fire Safety, First Aid/Choking and Standard Precautions.
  • Degree in the area of Social Services.
  • Certified as CNA if CNA.
  • Valid driver’s license and good driving record preferred
  • At least one year of experience working within a residential based or behavioral program.

Responsibilities

  • Functions at all times as a therapeutic agent by demonstrating teaching, reinforcing, anticipating, interacting, and noticing program participant behavior.
  • Tends to the program participant immediate needs for safety and performs all aspects of participant care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Provides treatment in accordance with unit and facility policies through active participation with residents during activities, ADL's, school, work, meals, groups, etc., in a way that fosters independent functioning on the part of the resident.
  • Displays age-appropriate interaction skills. Actively and professionally participates in exchange of information. Interacts with family members and others in a professional manner.
  • Demonstrates consistent and reliable documentation on Education forms in accordance with Individual Service Plan (ISP) and prescribed treatment interventions.
  • Documents changes and/or unusual behaviors/situations. Completes other unit paperwork (behavior plans, checklists etc.) as required.
  • Demonstrates flexibility and responsibility in scheduling, coverage, and provision of clinical services to meet the needs of the residents and families.
  • Participates activity in all assigned training programs to meet state and company requirements. Adheres to the administrative, clinical, and human resource policies of Lakeview Specialty Hospital & Rehab. Other duties as assigned.
