About The Position

Community Options, Inc. is hiring a part-time Community Integration Counselor in Drums, Pennsylvania. The counselor assists people with disabilities in learning physical, intellectual, emotional, and social life skills. The role provides individualized services to help participants manage and overcome challenges associated with living in the community. Starting pay is $17.25 per hour.

Requirements

High school diploma or GED.
Valid driver’s license with a satisfactory driving record.
At least two years of relevant experience supporting individuals with intellectual and developmental disabilities.
Ability to establish a comfortable and supportive relationship with individuals receiving support.
Successful completion of criminal background, central registry, child abuse registry, and drug testing checks is required for employment.

Bachelor’s degree preferred.

Responsibilities

Support and encourage individuals to increase their independence.
Meet regularly with waiver participants at their residences, in the community, or in an office.
Provide ongoing support for community integration efforts.
Document individuals’ progress toward community integration.
Help build support networks between individuals and the community.
Administer medications and complete required documentation.
Coordinate leisure and volunteer opportunities.
Monitor individuals’ health and medical needs and immediately report concerns.
Drive individuals to medical appointments and community activities.
Complete program documentation and billable records accurately and on time.
Cooperate with licensing and department staff during inspections, inquiries, or investigations.
Perform additional assigned tasks and responsibilities.
