About The Position

This role operates in a Supported Employment Day Habilitation (SE-HAB) Setting, defined as services “that enables the member to obtain and maintain individual employment” (Iowa Health & Human Services). Rather than assuming individuals with development or intellectual disabilities are not qualified to work, SE-HAB services provide these individuals with the opportunity to obtain and maintain meaningful employment that supports their life goals. Community Integration Coordinator support the mission of the organization – empowering people to reach individual achievement across the spectrum of life. As a Community Integration Coordinator, you’ll provide leadership to Community Integration Professionals, Environmental Associates, General Laborers, Production Associates, Clerical Assistants, and Assemblers. As an integral part of the overall supported employment team, you will collaborate with team members, individuals-served, and their families to ensure the best possible care for those you serve. You will creatively strategize ways for an individual to find employment that suits their skills, abilities, and goals.   • Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this.
  • Work Authorization: Candidates must be able to work in the U.S. without sponsorship

Responsibilities

  • Recruit, supervise, schedule, and develop Community Integration and production staff through coaching, training, team meetings, site visits, and active engagement while maintaining a positive team culture and ensuring adequate staffing coverage.
  • Oversee day-to-day operations, including vehicle scheduling, medication monitoring, financial oversight in partnership with the Representative Payee Team, appointment coordination, and management of resources necessary for quality service delivery.
  • Partner with individuals served, families, CBCMs, and the Interdisciplinary Team to develop, implement, and monitor individualized service plans, goals, assessments, and authorizations that support each person’s needs, preferences, and desired outcomes.
  • Foster relationships with community organizations and stakeholders, collaborate with the Strategic Advancement Team to promote services, and ensure individuals have access to transportation, supports, and opportunities that enhance community inclusion.
  • Maintain compliance with all applicable regulations, policies, and industry standards; ensure accurate documentation and incident reporting; and advocate for individual choice, dignity, independence, and self-determination in all aspects of service delivery.
