About The Position

On both an individual and group basis, the Community Inclusion Worker supports the quality of life of the persons we serve by ensuring that their physical, social, vocational, educational, and medical needs are met. The Community Inclusion Worker participates as part of the team responsible for implementing all aspects of the participant’s program, including agency policies, procedures, and philosophy.

Requirements

A Community Support Worker Certificate, or equivalent education and experience
Three months’ experience delivering similar services supporting individuals with disabilities in community settings
Valid First Aid and CPR certification
Valid Food Safety Certification
Driver's license
A current and satisfactory criminal record search and driver's abstract
A physician’s health reference
A negative TB test

Responsibilities

Ensuring that the physical, social, vocational, educational, and medical needs of participants are met.
Implementing all aspects of the participant’s program, including agency policies, procedures, and philosophy.
Providing personalized support and teaching life skills and self-advocacy skills to participants and families.
Working effectively with families, staff, volunteers, and organizations.
Working alone and unsupervised, as well as part of a team.
Performing lifts and transfers of clients.
