About The Position

Resources for Human Development (RHD) is seeking an energetic, compassionate, and community-focused Community Inclusion Specialist (CIS) to join their Growth Day Program team. This role supports adults with developmental disabilities, helping them build meaningful community connections, develop life skills, explore cultural opportunities, and achieve personal goals. The Growth Day Program supports individuals in achieving greater independence, community inclusion, employment success, and personal growth. Community Inclusion Specialists help individuals participate fully in community life through meaningful outings, cultural experiences, volunteer opportunities, recreational activities, and skill-building experiences. Every day offers an opportunity to help individuals develop confidence, independence, and stronger connections within their communities.

Requirements

High School Diploma
Valid Driver's License and clean driving record
Proof of active vehicle insurance
Three years of experience working with individuals with developmental disabilities
Basic computer skills, including Microsoft Office programs
Access to a reliable vehicle for work-related travel
Ability to obtain and maintain annual PMT certification
Ability to complete all required trainings and clearances
Ability to travel throughout the community daily
Strong communication and interpersonal skills

Experience supporting community participation and employment services

Responsibilities

Escort, supervise, and support groups of up to three participants during community outings and activities.
Create meaningful opportunities for community participation and engagement.
Facilitate outings that may include museums, parks, volunteer activities, cultural events, community programs, and life-skills activities.
Support individuals as they build connections and relationships within the community.
Encourage independence and active participation during community experiences.
Assist individuals with activities of daily living as needed.
Support individuals according to their Individual Support Plans (ISPs) and behavioral support plans.
Maintain knowledge of each individual's goals, preferences, strengths, and support needs.
Provide person-centered support that promotes growth and independence.
Assist with medication administration and basic first aid when required.
Develop and coordinate daily community outings and activities.
Create monthly outing budgets and maintain records of expenditures and receipts.
Identify opportunities that align with participants' interests and goals.
Ensure activities promote inclusion, community engagement, and skill development.
Complete daily progress notes and required program documentation accurately and on time.
Maintain records related to outings, expenditures, and participant progress.
Follow agency policies and procedures.
Maintain confidentiality and adhere to HIPAA requirements.
Attend required trainings and meetings.
Maintain annual PMT certification.
Utilize public transportation and program-approved transportation methods to support community participation.
Drive a personal vehicle as required to assist individuals in accessing community-based opportunities.
Ensure participant safety while traveling and participating in community activities.
