Requirements

  • High school diploma or GED required.
  • Two (2) years of work experience directly related to the duties and responsibilities, or equivalent combination of education, training, and experience preferred.
  • If not certified, will need to obtain CCHW within 6 months of employment.
  • Knowledge of community health resources and social determinants of health
  • Ability to effectively communicate with a diverse population and establish trust
  • Effective active listening skills
  • Excellent verbal and written communication skills
  • Experience using Microsoft Office and data entry
  • Ability to prioritize workflow and manage time
  • Ability to handle many tasks simultaneously and respond to customers and their issues promptly and professionally.
  • Ability to take direction and to navigate through multiple systems simultaneously.
  • If hired, as a condition of employment you must submit to a full background check and drug screening.

Nice To Haves

  • Certified Community Health Worker preferred but not required
  • Associate's degree in human services or another correlated field preferred.
  • Bilingual in Spanish (speaking, reading, and writing) a plus, but not required

Responsibilities

  • Builds relationships with assigned members to understand their concerns, needs, and works toward collaborative goal setting.
  • Provide educational material to members on programs provided.
  • Provide support and insight that encourages members to attend all care appointments.
  • Assist with scheduling appointments, reminder of appointments, and assisting with obstacles that may prevent attending appointments.
  • Conducts health risk assessments to identify member needs and provides education and additional resources.
  • May attend community meetings to understand community issues or to build relationships with community members on an as needed basis.
  • Assists members with culturally appropriate Social Determinants of Health (SDOH) needs.
  • Remains up to date on knowledge of community resources relating to various areas of specialty appropriate to the needs of members/families through participation in ongoing trainings, informal learning opportunities, supervisor mentoring, and general professional development opportunities.
  • Engage members in telehealth/videoconferencing to conduct home visits. In person home visits will be conducted only in special cases based on need with advance approval.
  • Adheres to weekly schedule to allow for optimal member outreach and documentation.
  • Attendance at business and department meetings.
  • Conducts additional work assignments corresponding with department needs at the direction of supervisor/manager.
