About The Position

This full-time Community Healthcare Worker position is based in Fredericksburg, VA, within a high-volume practice that offers a rewarding, team-oriented work environment. The role primarily involves advocating for clients and the community, identifying and addressing health inequities, and connecting patients and families with essential social services and community resources. Key responsibilities include conducting client meetings in their homes and at community events, providing general wellness education, and transporting clients to health-related appointments. The position also requires diligent documentation in an electronic medical record system, assisting with appointment follow-up and scheduling, and maintaining necessary reports and data records.

Requirements

  • Valid High School Diploma or GED
  • Ability to work independently
  • Maintain confidentiality
  • Excellent Problem-Solving Skills
  • Written and verbal communication skills
  • Knowledge of community resources and experience with high-risk populations
  • Ability to work collaboratively with the community, patients, families and other members of health care teams
  • Valid Driver's License and ability to own insured vehicle
  • Familiarity of basic computer skills, word processing, and data entry
  • Experience with basic office skills such as filing, scheduling, and customer service
  • Ability to obtain BLS certification

  • Willingness to obtain state CHW certification

Responsibilities

  • Advocate for your client and community
  • Identify and help address health inequities and barriers to good healthcare
  • Meet with clients in their homes and at community events
  • Provide social service referrals
  • Provide general wellness education
  • Transport clients to health-related appointments
  • Connect patients and families with community resources
  • Follow up with clients and agencies as appropriate to document use/success of referral
  • Document in the electronic medical records
  • Assists with follow up and scheduling of appointments as needed
  • Completes necessary reports and maintains records of data as required
  • Performs other duties as assigned
