About The Position

This is a full-time position for a Community Healthcare Worker at CVHS Fredericksburg in Fredericksburg, VA. The role involves advocating for clients and the community, identifying and addressing health inequities, meeting with clients in various settings, providing social service referrals and wellness education, and transporting clients to appointments. The position requires knowledge of community resources, connecting patients with these resources, and following up on referrals. Responsibilities also include documenting in the electronic medical records (eCW), assisting with appointment scheduling, and completing necessary reports.

Requirements

  • Valid High School Diploma or GED.
  • Ability to work independently.
  • Maintain confidentiality.
  • Excellent Problem-Solving Skills
  • Written and verbal communication skills.
  • Knowledge of community resources and experience with high-risk populations.
  • Ability to work collaboratively with the community, patients, families and other members of health care teams.
  • Valid Driver's License and ability to own insured vehicle.
  • Familiarity of basic computer skills, word processing, and data entry.
  • Experience with basic office skills such as filing, scheduling, and customer service.
  • Ability to obtain BLS certification.
  • Willingness to obtain state CHW certification.

Responsibilities

  • Advocate for your client and community.
  • Identify and help address health inequities and barriers to good healthcare.
  • Meet with clients in their homes and at community events.
  • Provide social service referrals.
  • Provide general wellness education.
  • Transport clients to health-related appointments.
  • Connect patients and families with community resources.
  • Follow up with clients and agencies as appropriate to document use/success of referral.
  • Document in the electronic medical records.
  • Assists with follow up and scheduling of appointments as needed.
  • Completes necessary reports and maintains records of data as required.
  • Performs other duties as assigned.
