About The Position

UNM Medical Group, Inc. is hiring for a Community Health Worker to join our Truman Health Services Clinic. This opportunity is a full-time day shift opening located in Roswell, New Mexico. This is a term position with an end date of 9.30.2028. The salary is determined based on years of total relevant experience and is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE. Truman Health Services provides patient-centered medical care for HIV and gender-affirming primary care, with consult services for Hep-C and PrEP. Requirements

High School diploma or GED.
Must complete New Mexico State CHW Certification within 1 year of employment.
Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
Verification of education and licensure will be required if selected for hire.
Possession of a valid unrestricted NM driver's license.

Responsibilities

Assists clients in their homes, community, or clinic setting.
Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing.
Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
Assist in identifying individuals who would benefit from retention in care strategies.
Coaches patients in effective management of their chronic health conditions and self-care.
Assists patient in understanding care plans and instructions.
Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
Assists clients with overcoming barriers to obtaining needed medical care and /or social services.
Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed.
Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.
Works to reduce cultural and socio-economic barriers between clients and institutions.
Participate in community events, health fairs, and school events to engage individuals to participate in retention in care clinics.
Provide clear, understandable, current, and accurate health related information to clients; provide information and referrals in a way that demonstrates respect, active listening skills, empathy, and patience.
Maintain professional growth and development through completion of required departmental competencies.
Perform miscellaneous job-related duties as assigned.
