About The Position

Light of Life Rescue Mission is a safe refuge for homeless and hurting people in the Pittsburgh area. We are seeking a Community Health Worker to be a part of the Mission's collaborative and comprehensive approach to healthcare and wellness. This individual plays a vital role assisting those we serve navigate the healthcare and social services system. Light of Life is nearing completion of a Capital Campaign and relocation plan which will provide the successful applicant an opportunity to be a part of exciting growth at the Mission.

Requirements

  • Must have own, reliable transportation as travel between Light of Life buildings will be required.
  • Detail oriented; finds satisfaction in accuracy and process.
  • Strong interpersonal skills; adept at building relationships with diverse groups.
  • Calm, pleasant demeanor; able to create a safe and comfortable environment for those needing assistance.
  • LPN or RN Certification
  • Associate degree in psychology, social work, health administration or a related field.
  • CPR certified.
  • 2-4 years' work experience in a clinical or human services setting.
  • Strong interpersonal and communication skills, collaborative, able to work effectively with diverse groups of people.

Nice To Haves

  • Knowledge of recovery, addiction, mental health preferred.
  • HMIS experience preferred.

Responsibilities

  • Help guests and clients interact with external health care/social service systems.
  • Educate guests and clients regarding their health condition(s) while developing ways to improve their health and lifestyle.
  • Influence program participants to sustain healthier behaviors and lifestyle choices.
  • Collaborate with programs leadership regarding health protocols and client care.
  • Deliver health information and concepts that will be culturally acceptable.
  • Provide health care resources to individuals.
  • Provide influence with informal counseling, support, and follow-up.
  • Administer health services, such as blood pressure monitoring or first aid.
  • Translating and interpreting health care/social service information made available by providers.
  • Act as a liaison between Light of Life and outside community partners on community health-related topics.
  • Help educate clients, guests, and Light of Life staff on medications and how to manage them properly.
  • Oversee the Lab Technician and support the role as needed.
  • Walk alongside clients as a disciple of Christ and share the Gospel with them through actions and words.
  • Establish mutual trust and create a safe, authentic environment.
  • Serve clients and co-workers in a Christlike manner and practice a growth-oriented lifestyle.
  • Help the clients make choices that are justifiably in their eternal best interest.
  • Practice stewardship of all resources.
