About The Position

The Community Health Worker helps connect people to services at the JFS health centers. This role engages community members, connects patients to care, identifies health and social needs, provides referrals, and supports patients in navigating healthcare and community resources.

Requirements

  • High school diploma or GED required.
  • Community Health Worker certification required (or obtained within 6 months of hire).
  • Strong communication, relationship-building, and customer service skills.
  • Ability to work effectively with diverse populations in a respectful and patient-focused manner.
  • Ability to maintain accurate records and manage outreach, referral, and follow-up activities.

Nice To Haves

  • Experience in community outreach, patient engagement, healthcare, social services, or a related field preferred.
  • Experience with screenings, referrals, intake, or resource navigation preferred.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Engage community members and partner organizations to promote JFS and recruit patients for JFS health centers.
  • Conduct screenings and assessments to identify health and social needs.
  • Educate patients about available services and connect them to care.
  • Follow up with patients to encourage appointment scheduling, attendance, and continued engagement.
  • Assist patients with registration, intake, consent forms, and income verification when needed.
  • Provide referrals to JFS programs and community resources.
  • Complete appointment scheduling and provide care coordination.
  • Assist patients with applications for public benefits and other support services.
  • Follow up on referrals and appointments to reduce barriers to care.
  • Provide basic health education on preventive care, chronic disease management, healthy lifestyles, and community resources.
  • Support health education events, outreach activities, and community programs.
  • Maintain accurate records of outreach, screenings, referrals, and patient interactions.
  • Track outreach activities and referral outcomes and assist with reports.
  • Distribute program incentives, such as gift cards, according to guidelines.
  • Perform other duties related to patient outreach, navigation, and engagement.
