About The Position

Under the supervision of a Care Manager, the Community Health Worker provides essential administrative, logistical, and direct-support assistance to facilitate the delivery of care management services to CALAIM authorized members. The CHW supports the care management team in coordinating community-based services, conducting outreach, processing referrals, and ensuring clients receive timely and dignified access to the programs and resources they need. This role requires compassion, attention to detail, strong organizational skills, and the ability to work effectively with a diverse population.

Requirements

  • High school diploma or GED
  • Prior experience in community social services, health education and care coordination.
  • Minimum 1-2 years of experience in a social services, health care, or community-based organization setting.
  • Experience working with adults.
  • Experience working with underserved, low income or culturally diverse populations, lived experience
  • Knowledge of community resources, social service programs, and public benefits available to adults in Santa Clara County.
  • Ability to communicate clearly and empathetically with clients, families, and service providers from diverse backgrounds.
  • Strong organizational skills with the ability to manage multiple cases and deadlines simultaneously.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to learn case management software.
  • Ability to maintain confidentiality and adhere to HIPAA and other privacy regulations.

  • Bilingual proficiency in Russian, Spanish, Vietnamese, Mandarin, Cantonese, or other language(s) reflecting the county's population is highly desirable.

Responsibilities

  • Contacting members who may need care coordination, following up on gaps in care, and helping hard-to-reach populations engage with services.
  • Monitor client well-being through regular phone or in-person check-ins and report changes in status to the supervising Care Manager.
  • Working alongside care managers and nurses to support members with chronic conditions, complex needs, or social determinants of health (SDOH) barriers.
  • Assist Care Managers in implementing individualized care plans for clients.
  • Accompany Care Managers on home visits and community assessments as needed; may conduct follow-up visits independently under supervision.
  • Connecting members to community resources such as food assistance, housing, transportation, and behavioral health services.
  • Schedule and coordinate client appointments, home visits, and service provider contacts.
  • Provide clients and caregivers with information about available community resources, benefits programs, and county services.
  • Coordinate linkage to services including IHSS, Meals on Wheels, transportation, adult day programs, mental health services, housing support, and other community-based programs.
  • Providing culturally and linguistically appropriate health education to members and their families.
  • Documenting outreach attempts, updating member records, and tracking referrals in the care management system.
  • Assist clients in completing applications for public benefits, including Medi-Cal, CalFresh, Medicare Savings Programs, and other entitlements.
  • Serving as a liaison between the health plan, providers, and the community, particularly for underserved or non-English-speaking populations.
