About The Position

Community Health Workers (CHWs) are trusted members of communities they serve who build relationships at the grassroots level and support individuals, families, and communities by ensuring they are connected to the health and social services they need to be healthy and well. As a CHW, this is an opportunity to work alongside communities and service providers to improve cancer-related health outcomes among those who are most impacted by health disparities such as people of color, those with lower income, and people with low access to healthcare. The Louisiana Public Health Institute (LPHI) is a statewide, non-profit organization that has been promoting the health and well-being of Louisianians since 1997. A successful candidate will have a strong connection with and regional expertise of priority communities in Louisiana. Day-to-day responsibilities may include managing schedules and patient appointments, working directly and communicating with patients in a community or clinical setting, working closely with clinical staff, managing referrals, identifying, and prioritizing items for attention, documentation management, and program activity coordination. Requirements

  • 3-5 years of experience as a Community Health Worker, supporting community care coordination required.
  • Knowledge of public health, chronic conditions, and regional healthcare navigation.
  • Strong interpersonal and relationship-building skills with experience working with individuals and groups of diverse backgrounds.
  • Ability to think proactively, be a self-starter, and function both independently and as a team.
  • Ability to collaborate effectively and build strong, positive relationships with colleagues and partners, both internal and external to LPHI.
  • Exceptional emotional intelligence, including interpersonal skills and experience working with individuals and groups from diverse backgrounds.
  • Demonstrated learning orientation; ability to learn new subject areas quickly.

Responsibilities

  • Conduct outreach activities to educate community members about preventive health measures and available services using, virtual, in-person, or hybrid platforms.
  • Assist individuals in navigating the healthcare system, including helping them access and make referrals for medical care, social services, and emergency assistance.
  • Provide support to individuals managing chronic conditions and advocating for their needs.
  • Support documentation management efforts to gather and relay community health data back to healthcare organizations to ensure that programs and policies are responsive to community needs.
  • Communicate and coordinate across internal teams and with external partners to gain an understanding of internal and community assets to broaden perspective, and to continue to grow and learn.
  • Coordinate and facilitate meetings, trainings, and events using virtual, in-person, or hybrid platforms as required.
  • Develop communications materials, including e-mails to partners, website updates, promotional products, and more.
  • Proactively collaborate with team members to ensure tasks are completed.
  • Participate in organization, department, and project level trainings and meetings.
  • Exercise good judgment with respect to priorities and effective use of the team’s time.
  • Develops a deep understanding of the programmatic areas and partners to support administrative improvements.
  • Maintain files of relevant organizational and programmatic documents. Updates documents, as necessary.
  • Engage in professional development activities to improve job-related skills and knowledge.
