Requirements

  • New Community Health Workers (CHWs) will be required to attend the New Jersey Department of Health, Colette Lamothe Galette Institute training and coursework
  • Candidate must be fluent with proficiency in the following language(s): Spanish and English
  • Candidate must be a resident of New Jersey.
  • Must have shared life experiences in the communities they serve with shared values to support as well as become an intermediary link between health/social services and the community in order to facilitate access to services and improve the quality of service delivery system.
  • Understand the impact of cultural diversity, cultural humility and cultural responsiveness training
  • Promote an environment of care, which is inclusive and respectful of every internal and external stakeholders
  • Effective problem-solving, organizational, and time management skills
  • Decision-making, priority setting, and work delegation abilities
  • Administrative/Technological skills, including Microsoft Office (Excel, Word, PowerPoint, Access), and Outlook
  • Excellent knowledge of available community services and resources

  • Must have a Valid NJ drivers’ license and own an insured car; position requires some travel within the counties
  • Must be able to work legally in the United States

Nice To Haves

  • Associate’s or Bachelor’s degree in related field preferred
  • Prior and relevant professional experience working in the community
  • Internships may be substituted as work experience
  • Proof of completion of Community Health Workers training (New Jersey Department of Health Colette Lamothe Galette Institute, IMPaCT Care or relevant certification).

Responsibilities

  • Work with CHW Program Supervisor and Local Health Department (LHD) to perform outreach activities and help to formulate strategies in order to communicate information about COVID-19 and health/social issues to migrants/seasonal workers and immigrants in Hudson County
  • Conduct phone and in person interviews and informational sessions with migrant/seasonal workers and immigrant community members
  • Assist in conducting direct outreach to migrant/seasonal workers and immigrant communities, local churches and other organizations that serve the immigrant population
  • Help to organize and host virtual and in-person community forums on COVID-19 and social determinants of health, risk factors and protective factors
  • Help the supervisor develop and share outreach and information materials with local communities and on social media
  • Meet regularly with the CHW Project Supervisor and attend meetings with Local Public Health designated staff, when applicable
  • Document all interviews/information sharing contacts in project database to create an ongoing record of each case and provide detailed information for monthly metrics reports required by grantor (Unite Us and Local Public Health Database)
  • Assist in the preparation of monthly narrative and statistical reports regarding outreach and client counseling efforts to be delivered to the funders
