About The Position

Our Community Health Worker helps increase access to health services for people through outreach, encouraging engagement, education, and peer support. This role involves conducting patient and family assessments to identify needs such as food, housing, transportation, financial stability, and access to healthcare, as well as identifying gaps in care. The Community Health Worker is responsible for documenting assessments and interactions, coordinating outreach efforts through various channels, and supporting patients in managing chronic conditions and engaging in preventative care. Collaboration with primary care providers, care managers, and support staff is crucial to improve patient outcomes. The position also entails addressing barriers to care, assisting patients in accessing timely services, educating them on available resources, connecting them with community services, and building partnerships with community organizations.

Requirements

  • Completion of Community Health Worker (CHW) Training required
  • Seasonal Flu Vaccine
  • Experience working with diverse populations and adapting communication styles as needed
  • Compassionate and patient-focused approach
  • Ability to work both independently and as part of a team
  • Strong time management and adaptability in a fast-paced environment

Responsibilities

  • Conduct patient and family assessments to identify needs such as food, housing, transportation, financial stability, and access to healthcare
  • Identify gaps in care, including missed primary care visits, preventative care needs, and inappropriate ER use
  • Document assessments and interactions in the patient record
  • Coordinate outreach efforts using phone calls, text messaging, mailings, home visits, and community events
  • Encourage and support patients in managing chronic conditions and engaging in preventative care
  • Collaborate closely with primary care providers, care managers, and support staff to improve patient outcomes
  • Address barriers to care and assist patients in accessing timely services
  • Educate patients on available resources, including financial assistance, insurance programs, and sliding fee discounts
  • Connect patients with community services and support programs
  • Build and maintain partnerships with community organizations
