About The Position
Project FoodBox is a Food as Medicine company that partners with health plans, community organizations, and local farms to deliver medically tailored groceries to individuals with chronic conditions and food insecurity across California and New York. Since 2020, Project FoodBox has delivered nearly 10 million boxes, totaling over 250 million pounds of fresh food. The company is experiencing rapid growth and is seeking individuals to help build the systems, processes, and infrastructure for national expansion. The Community Health Worker role is crucial for driving member enrollment at community events and acting as a trusted, culturally responsive presence within the communities served. This role involves directly engaging prospective members, educating them on the benefits of medically tailored meals for managing chronic conditions, screening for eligibility, and completing enrollments on-site. Performance will be evaluated based on completed enrollments, submission accuracy, and the quality of leads provided to outreach coordinators. Community Health Workers will collaborate with outreach coordinators who manage events and partnerships.
Requirements
- Community Health Worker certification is required.
- Comfort starting conversations with strangers and asking eligibility and enrollment questions confidently.
- Ability to handle sensitive health information with discretion and maintain confidentiality.
- Attention to detail when completing enrollment forms and documentation.
- Availability to work flexible hours, including evenings and weekends, and to stand for extended periods with light lifting.
Nice To Haves
- Bilingual in Spanish and English.
- Lived experience in or a strong connection to the communities we serve.
- Experience as a Community Health Worker, promotor(a), or in enrollment, outreach, or event staffing.
- Familiarity with Medicaid or health plan enrollment processes.
Responsibilities
- Engage attendees proactively, screen for eligibility, and complete on-site enrollments.
- Explain how medically tailored meals support chronic condition management, along with eligibility and next steps, in plain and accurate language.
- Serve as a trusted liaison between Project FoodBox and the community, meeting people with empathy and reducing barriers to access.
- Recognize other needs that surface in conversation and connect members to appropriate community resources.
- Capture contact information from prospects who don't enroll on-site and hand qualified leads to outreach coordinators.
- Complete enrollment forms and documentation accurately so no member is lost between the event and follow-up.
- Support event setup, breakdown, and on-site logistics, and protect member privacy under CMS and HIPAA requirements throughout.
