About The Position

The Community Health Worker (CHW) will educate patients about relevant community resources, help them set health-related goals, and assist with accessing transportation for appointments. This role involves navigating patients through the care continuum, including appointment scheduling, visit completion, and follow-up coordination. The CHW will also provide educational lectures on health-related topics, collaborate on program strategy and roll-out, and report on program barriers and successes. Additionally, the CHW will complete Spanish interpreter certification, provide language translations, and assist community members in finding services such as rent relief, housing, healthy food, and energy assistance, as well as help with FAP or Medicaid applications.

Responsibilities

Educate patients about relevant community resources appropriate to their needs.
Help patients set personal health related goals.
Assist patients access transportation for appointments.
Navigate patients through the care continuum including appointment scheduling, visit completion, follow-up coordination and verification with the physician that the patient understood the results of the visit.
Provide educational opportunities via lectures on health-related topics.
Collaborate on strategy and program roll-out, report on barriers and successes of the program to the CHW program coordinator/manager.
Complete Spanish interpreter certification.
Provide language translations at the hospital and in the community to help access medical resources.
Assist community members find services such as rent relief, housing, healthy food and energy assistance.
Assist community members with applications for FAP or direct them to resources for Medicaid application.
