Community Health Worker
About The Position
Horizon Health in Mission, SD is seeking a full-time Community Health Worker to educate and connect patients who have identified social determinants of health with available resources and assist them in navigating barriers. The position supports high-quality, patient-centered care in a collaborative, team-oriented environment focused on individual patient needs and community health and wellness. Responsibilities
- Navigate the health system and coordinate resources.
- Provide health promotion and coaching, sharing information or education that positively contributes to patients’ health status.
- Teach or promote methods and measures shown to be effective in avoiding illness or lessening its effects.
- Provide culturally appropriate health information and education to patients.
