Community Health Worker

CVS Health•Toledo, OH
•$19 - $35

About The Position

A Community Health Worker (CHW) at Oak Street Health acts as a bridge between patients, the community, and medical systems to remove barriers and improve wellness across all life domains. The CHW advocates for and liaises with patients through proactive in-person and phone outreach, promoting health literacy and connecting patients with resources that support healthier lives. The role requires flexibility, problem-solving, strong communication, and knowledge of the community served. No clinical license is expected, and CHWs are expected to work within their scope of practice. CHWs work closely with Medical Social Workers to manage patient care plans, support care-team decisions, and coordinate clinical and complementary services. They meet patients in preferred locations, including health centers, homes, medical facilities, and community settings. About half of the role is spent outside the center in patient-centered locations, so reliable transportation is required. The position is full-time, with 40 anticipated weekly hours.
