About The Position

Southern Illinois University Edwardsville (SIUE) is seeking an Extra Help - Community Health Worker (EH26-114) to support the Asthma Trigger Assessment Program. This role involves direct work with individuals and families affected by asthma, focusing on identifying environmental triggers, providing education, connecting patients to resources, and reducing asthma-related complications through community-based outreach. The Community Health Worker acts as a liaison between patients, families, healthcare providers, and community resources to foster healthier home environments and improve asthma self-management.

Requirements

  • High school diploma or equivalent required
  • Valid Illinois Driver’s License
  • Reliable transportation and willingness to travel to homes and community locations as required
  • Basic computer skills and ability to complete documentation in REDCap
  • Application must include a complete and detailed employment history highlighting relevant full-time and part-time work experience, including start and end dates of employment, and at least three professional references.
  • Out-of-state candidates must establish Illinois residency within 180 calendar days of the start date for this position.

Nice To Haves

  • BSPH or MPH in Public Health preferred
  • Experience in community outreach, health education, social services, public health, or a related field preferred
  • Knowledge of asthma, chronic disease management, or home environmental health preferred
  • Strong communication and interpersonal skills
  • Strong organizational skills and attention to detail
  • Ability to work independently and as part of an interprofessional team

Responsibilities

  • Conduct home visits as part of the asthma trigger assessment program
  • Identify environmental asthma triggers in the home, such as dust, mold, pests, smoke, strong odors, and other irritants
  • Complete chart assessments and program documentation in the REDCap platform accurately and on time
  • Provide education to patients and families on asthma management, trigger reduction, medication adherence, and when to seek medical care
  • Reinforce the asthma action plan and help patients and families understand provider recommendations
  • Assist with distributing program supplies and materials, such as cleaning supplies, mattress covers, pillow covers, dehumidifiers, air quality support items, or other approved resources
  • Connect families to community resources that address social needs affecting asthma outcomes
  • Work collaboratively with the healthcare team, including nurse practitioners, nurses, asthma coordinators, and other program staff
  • Follow up with patients and families to assess progress, barriers, and ongoing needs
  • Maintain confidentiality and protect patient information in accordance with program, institutional, and legal requirements
  • Participate in team meetings, training sessions, and program quality improvement activities
  • Support outreach and recruitment efforts for the asthma trigger assessment program as needed
