About The Position

The Community Health Worker assists UTMB Health patients and their families by addressing health and social challenges and educating the community about the diverse programs and services UTMB provides.

Requirements

  • Associate degree with two (2) years of experience or equivalent in healthcare.
  • Valid driver’s license.
  • Good written and verbal communication skills in both English and Spanish.

Nice To Haves

  • Bilingual highly desirable.

Responsibilities

  • Serve as the primary point of contact for patient and community outreach activities related to a specific program.
  • Educate the community and patients about a program and available services.
  • Develop relationships with community organizations, local outreach programs, and other patient- or program-specific initiatives.
  • Assist and educate patients in navigating the healthcare system (including scheduling and keeping appointments, securing transportation, communicating with providers, and accessing community resources).
  • Serve as a resource for patients and families by providing access to health information and community resources and addressing barriers to care.
  • Communicate with healthcare providers about patient-specific health status as needed to ensure continuity of care.
  • Create and implement community-wide marketing campaigns, including the development of marketing materials.
  • Manage content for program-specific websites and social media accounts.
  • Travel during business hours for patient care and community outreach needs when required.
  • Adhere to internal controls and reporting structure.
  • Perform related duties as required.
