The Community Health Worker (CHW) is a frontline public health worker who serves as a trusted liaison between patients, families, UMMA Health, and community-based services. Drawing upon lived experience, cultural understanding, language skills, or close knowledge of the communities UMMA Health serves, the CHW helps patients overcome barriers to care, navigate health and social-service systems, and build the knowledge and confidence needed to achieve their health goals. Working as a nonclinical member of the interdisciplinary care team, the CHW provides culturally and linguistically responsive outreach, health education, social support, service navigation, advocacy, and follow-up in clinics, community, home, and other approved settings.
