About The Position

The Community Health Worker (CHW) supports individuals experiencing homelessness or housing instability by providing outreach, care coordination, and assistance connecting to healthcare and community-based services. Personal Statement and Resume Required for consideration. Applicants must submit a 500 word or less Personal Statement describing their connection to and understanding of the population served.

Requirements

  • Demonstrated lived experience or sustained professional, volunteer, or community-based experience working with individuals or communities experiencing homelessness or housing instability
  • Valid Driver's License and reliable transportation to travel throughout Davidson County.

Responsibilities

  • Conduct outreach, perform intake assessments, and screenings to identify patient needs, eligibility, and barriers to care.
  • Provide person-centered support to help patients establish health-related goals and develop actionable plans to achieve them.
  • Assist patients in navigating healthcare and community systems, including selecting primary care providers, scheduling services, and coordinating referrals.
  • Conduct direct outreach and engagement services to assigned individuals, connecting them to housing, healthcare, and community-based resources.
  • Collaborate with interdisciplinary teams and community partners to coordinate care, support service delivery, and maintain continuity of care.
  • Document encounters and activities accurately and maintain required records to facilitate care coordination, reporting, and program evaluation.
  • Participate in team meetings, training sessions, outreach activities, and quality improvement initiatives to enhance CHW practices and program effectiveness.
